Items to Bring to Your Appointment

1. All insurance cards.

2. Photo ID (e.g., driver’s license, government issued ID etc.)

3. List of all medications you are taking, including over the counter. If this includes blood thinners, bisphosphonates (e.g. boniva, fosamax, actonel etc.), please let our office know at least a week prior to your appointment.

4. Original documents relating to adoption, guardianship, or power of attorney, if applicable.

5. A form of payment for treatment, including co-insurance and deductibles. We accept most commercial insurance plans, including but not limited to Delta Dental, Healthy Kids Delta Dental, MiChild, Blue Cross Blue Shield Dental, Cigna Dental, and MetLife Dental. We will be happy to provide an estimate of insurance coverage for your particular treatment. Please call our office to discuss payment plan arrangements prior to your appointment!

6. Please complete the online pre-registration form at least 1 to 2 weeks prior to your appointment. Doing so will allow us to review your insurance coverage and provide a more accurate estimated co-insurance.

7. Please contact us immediately for instructions if you develop a cold, sore throat, fever blisters, sinus infection, etc.

8. Children under 18 or individuals who are not responsible for their own medical decisions must be accompanied by a parent or legal guardian.
